The Extension School is committed to creating an accessible academic community where students with disabilities have equal opportunity to participate in, contribute to, and benefit from its academic programs. Accessible classrooms, reformatted course materials, extended time for classroom exams, notetakers, and computer access real time (CART) services for hearing impaired students are among the accommodations available. An adaptive technology laboratory at 53 Church Street is equipped with software and hardware to assist students with hearing and vision disabilities, repetitive stress injuries, and other disabilities.
All services and accommodations are coordinated and approved through Academic Services. The disability services coordinator works with students, faculty, and staff to identify and provide appropriate support services. Students must provide recent diagnostic documentation of the disability and submit a request for accommodation form for each course in which an accommodation is requested. Students should call, write, or fax the disability services coordinator to discuss their requests for accommodations before classes begin. Requests for American Sign Language interpreters must be made at least three months before the start of the term.
Requests for Accommodation
Students must follow the steps listed below when requesting accommodations. They are advised to call, write, or fax the disabilities services coordinator to finalize their requests for accommodations before classes begin.

2. Request for Accommodation Form
Students must complete Part I of the Request for Accommodation Form for each course in which academic accommodations are requested. Only one form is necessary from students who request on-campus transportation accommodations. Accomodation request forms may be downloaded from this website. (To download PDFs students must have Adobe's Acrobat Reader installed on their computers.)
Students should notify the disability services coordinator before classes begin concerning their adaptive equipment needs and any personal equipment they plan to bring with them. Please note that some types of equipment may not be available on-campus.
3. Schedule a Personal Interview
Students should make an appointment to speak with the disability services coordinator in person to finalize their accommodations.
Student's Rights
Grievance procedures exist to investigate and resolve specific allegations of discrimination or harassment.
